Appointments
Before Your 1st Visit
You will be asked to arrive a few minutes early to complete forms. You are responsible for payment at the time of service. If you would like us to submit a claim for insurance, we will be happy to do so. Please make sure to bring proof of your current insurance.
Initial Evaluation
Please allow between 1 to1 1/2 hours for the first visit. If you have information from your referring physician, please bring a copy to the visit. You may be asked to sign a release of information that will allow Dr. Cruz to communicate with other members of your health care team.
Subsequent Visits (Cancellation Policy)
Your appointment is time set aside for you. If for some reason, you must change your scheduled appointment, we ask that you provide us at least 24 hours notice so that we may accommodate another person during your appointment time. If you do not provide us at least 24 hour notice you may be charged for the missed visit (and this is not eligible for insurance coverage). If you must cancel a scheduled appointment after office hours please call the answering service at 828-258-4671 and ask the staff to notify Dr. Cruz between 7AM and 10PM.
Insurance
Blue Cross/Blue Shield Dr. Cruz accepts Blue Cross/Blue Shield but is not a preferred provider for varios PPO Plans.
Medicare Dr. Cruz currently accepts assignment for Medicare.
Cigna Dr. Cruz often is extended ad hoc coverage under Cigna but he is not a panel provider.
United Health Care Dr. Cruz is a member of the United Health Care network of providers.
Medicaid Dr. Cruz does not accept patients whose primary coverage is Medicaid.
Other Private Insurance Dr. Cruz accepts various other insurance coverage. You are urged to check with your insurance carrier to verify the extent of your coverage.
Our staff are glad to assist you with determining the benefits your insurance provides for mental health coverage. Your are encouraged to inquire prior to your first visit.
We recognize that emergencies can and do occur but if you are unable to keep an appointment please be considerate and call the office and provide adequate notice or you may be charged for the visit.
